Annual Health and Medical Record
Every Scout and adult participating in any Troop 990 program must complete a Health and Medical Record annually.
Completed Health and Medical Record forms should be emailed to the Troop 990 Advancement Coordinator advancement@troop990.org.
All Scouts and adults must complete Parts A and B of the BSA Health and Medical Record annually. Parts A and B are the health history and release sections and do not require an exam by a physician.
Any Scout or adult who will be camping for more than 72 consecutive hours, typically summer camp, must also complete Part C which requires an exam by a physician.
High adventure participants for Philmont, Northern Tier, Sea Base, or Summit Bechtel Reserve only must complete Part D as part of the physician exam.
